  • Role Type: Permanent Part time, working 16 hours per week

  • Department and Location: Towards Zero Suicides Team Service, St Vincent’s Hospital Darlinghurst

  • Salary: $73,427 - $104,752 (Pro-Rata) + Super + Salary Packaging

About the Role:

  • Permanent, part-time providing comprehensive social work services, including psychosocial assessments and risk evaluation, to patients, families, and carers in the Towards Zero Suicides Team Service.

  • Develop and implement individualised treatment and care plans in collaboration with the multidisciplinary team.

  • Lead safe discharge planning, coordinating referrals to community services and accommodation placements.

  • Deliver counselling to individuals and families, supporting adaptation to medical and lifestyle changes, grief, and loss.

  • Advocate for patients and carers with relevant government and NGO agencies, provide consultation and support, liaise with internal and external services, maintain accurate clinical documentation, and actively participate in multidisciplinary team activities.


About You:

  • Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW)

  • Thrive in suicide prevention settings, working side-by-side with Lived Experience Peer Workers to deliver fast, focused clinical assessment and care planning.

  • Confident managing a caseload, quickly identifying support needs, and stepping people up to crisis care or down to community services.

  • Driven by trauma-informed, person-centred care that’s culturally safe, non-clinical, and genuinely welcoming.

  • Skilled in co-creating and reviewing care plans that reflect lived experience input and real-world needs.

  • A natural collaborator who builds strong, purposeful partnerships across services to ensure seamless, wraparound support.
