The HVTI International Coordinator manages the intake and care coordination process for international patients seeking treatment within the Heart & Vascular Institute. Serving as the first point of contact, the coordinator facilitates the collection and review of medical records, schedules initial consultations, and ensures each patient is smoothly integrated into the appropriate care pathway.  

 

This role works closely with Global Patient Services (GPS) and HVTI providers, including direct collaboration with Dr. Maan Fares, Chairman of GPS, to ensure all clinical documentation is complete and properly routed. While most patient interaction is conducted via phone, occasional in-person support within GPS facilities may be required. 

 

A caregiver in this position works M-F, from 8:00am—5:00pm.   

 

A caregiver who excels in this role will:  

  • Coordinate the intake process for surgical candidates by obtaining, in advance, clinical information relative to the patients' state of health and level of functioning. 

  • Ensure departmental compliance with regulatory and The Joint Commission requirements. 

  • Coordinate the continuity of care of international/GPS patients, works with the PCP's, referring physicians, patients, Cleveland Clinic Cardiology staff, staff surgeons and their office staff, other nurse practice managers NPM) as well as the schedules officers, preop/TCI/postop nurse clinicians, to ensure continuity of care and appropriate concern for individualized pre-surgical and/or post-surgical needs. 

  • Review all clinic information available for review with surgeon for accurate and appropriate pre-surgical test and appointment scheduling, communicate additional studies to the outpatient scheduling office under the physician's direction. 

  • Coordinate patient care between HVI medicine and surgical departments when necessary. 

  • Facilitate hospital transfers in collaboration with scheduling officers, access plus and surgeons' office. 

  • Communicate anticipated patient care expectations to GPS. Identifies the appropriate Physician Champion to help streamline and oversee patient care. 

  • Other duties as assigned. 

Minimum qualifications for the ideal future caregiver include:  

  • Bachelor of Science in Nursing (BSN) from an accredited school of professional nursing 

  • Current state licensure as a Registered Nurse (RN) 

  • 4 years of experience as a nurse in medical cardiology or cardiac surgery clinical areas
