Job Description Summary:

This position is responsible for building and maintaining positive relationships for the Inpatient Rehabilitation Unit. He/She communicates and markets information pertaining to the unit and is responsible for the maintenance and participation of community involvement. This position is responsible for coordinating clinical and financial outcomes of the Rehabilitation population. He/She works collaboratively with interdisciplinary staff internal to OhioHealth and also external organizations to achieve timely, cost efficient and effective management of patient care. Ensures specific patient outcomes are achieved.

Responsibilities And Duties:

1. Assesses patient appropriateness for acute inpatient rehabilitation. Travels to locale hospitals for evaluation of patients2. Performs pre-certification of patients with insurance companies; verifies benefits and coverage for rehab patients; and negotiates per diem rates with such companies3. Maintains primary communication lines with team members, physicians, registered nurses, social workers, case managers, and all other affected parties4. Markets Rehabilitation Unit for hospital by building and maintaining positive relationships and referral sources5. Attends weekly team meetings regarding rehab unit patient progress6. Coordinates transfers with Charge Nurses and Unit Clerks7. Participates in community involvement to increase public awareness of rehabilitation unit and issues and concerns8. Conducts family tours of unit9. Coordinates multidisciplinary plan of care for patients in a case load. Directs and collaborates with the multidisciplinary team10. Monitors quality and type of services delivered to patients, ensuring management within established parameters11. Initiates and contributes to improved continuity and quality of patient care: increased participation and satisfaction of patients and families; efficient allocation of resources, appropriate length of stay and improved communication between disciplines.

Minimum Qualifications:

Associate's Degree (Required)RN - Registered Nurse - Ohio Board of Nursing

Additional Job Description:

Associates Degree; Licensed Nurse for the State of Ohio; RN from an accredited school of Nursing.Clinical expertise in care setting where position exists.Specialized Knowledge: Knowledge of computer applications for data inquiry, file access (lab values) and/or printing of radiology routine reportsKind and Length of Experience: 3-5 years of experience as Registered Nurse
