Starting Compensation:

22.02 - 24.00 USD Per Hour

Working Location:

Merced, CA

Working Hours/Shift:

Monday-Friday 830AM to 5PM

What you bring to SBHG:

  • Associate degree in a social science field required.
  • Bachelor’s degree in a social science field preferred.
  • Two (2) years direct service experience with the mentally ill, preferably adults required.
  • Valid CPR certification required within 30 days of employment. Valid First Aid certification required within 30 days of employment. Valid ProAct certification required within 30 days of employment. Valid California Driver's License required Meets Agency driver eligibility criteria required

How you will make a difference:

  • Identifies housing resources and developing relationships with community agencies.

  • Coordinates community outreach with outside agencies, and visit locations where clients are discharged to.

  • Conducts liaison work with potential employers and other employment service providers in the region.

  • Interprets facility policies and procedures to clients and family members.

  • Assists clients in completing facility consent forms upon admission.

  • Assists QA with client discharge satisfaction surveys and basis every 15 days.

  • Actively participates in treatment team meetings and is responsible for working with other disciplines to develop a coordinated treatment plan for assigned clients.

  • Provides consultation as needed with families and the Case Manager on issues regarding vocation, education, and housing.

  • Assists clients and families in utilizing available resources to meet client and family needs.

  • Facilitates resident council, community meeting and/or groups as instructed.

  • Transports clients to appointments to provide direct linkage to resources.

  • Assists clients in activity groups, encouraging appropriate behavior and providing feedback to clients.

  • 24/7 inpatient voluntary program
  • 16-bed facility for adults (ages 18-59)
  • Rehabilitation/recovery services, including substance use rehabilitation services
  • Pre-vocational or vocational counseling
  • Medication evaluation and support services
