Job Description:

Hospice RN Case Manager – Compassion at Every Mile

Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Service Area: Hastings up to Grand Rapids

How You’ll Make an Impact

As a Hospice RN Case Manager, you’ll empower patients and their families with expert care and compassionate guidance:

  • Holistic Assessments: Evaluate physical, emotional, and environmental needs to design personalized care plans.

  • Hands-On Care: Deliver attentive, evidence-based care during each visit, ensuring patient comfort and dignity.

  • Collaborative Communication: Serve as a liaison between physicians, clinical supervisors, and the care team to keep everyone aligned.

  • Education & Support: Provide families with the knowledge and tools they need to navigate the hospice journey with confidence.

  • Proactive Problem-Solving: Quickly respond to changes in patient conditions, ensuring timely adjustments to care plans.

  • Plan of Care Excellence: Follow individualized plans, ensuring each patient receives care tailored to their unique needs.

What We’re Looking For

To join us, you’ll need:

  • A nursing degree (Certificate, Diploma, Associate, or Baccalaureate) from an accredited program.

  • A current RN license in the state of practice.

  • At least one year of clinical experience (hospice or home health is a plus).

  • Strong organizational skills to manage multiple patients and priorities.

  • A compassionate approach to patient care.

  • Reliable transportation to travel
