Summary:

This role is the point of contact for new and existing patient referrals. Coordinates care to ensure the intake process is efficient and timely to meet the needs of internal and external clients. Serves as a liaison between the pharmacy, payors, patients and providers.

Responsibilities:

Triages new referrals, processes benefit investigation, prior authorization referrals and confirms medical necessity. Resolves complex third party rejections.Assists with collection and submission of medical information to confirm required for services is complete. Understands the process and requirements for therapies that require the use Durable Medical Equipment (DME) and the appropriate administrative supplies. Performs appropriate service follow ups and inputs missing information for newly admitted and existing patients.Supports staff training, transitioning and onboarding.Confirms and transcribes patient demographic, billing and other pertinent information into the department electronic medical record (EMR).Serves as the primary contact between internal and external customers to complete patient, medication and supplies referrals, including Durable Medical Equipment (DME) and related supplies.Communicates effectively in person, over the phone and in writing to provide customer service to the internal and external customers (patient, caregiver, physician, pharmacist) and liaisons between patient and providers for medication needs, including patient information, prescription processing, third party rejection resolution, delivery, storage, supplies, refills, education materials.Performs activities related to pharmacy technician scope of practice.Coordinates and facilitates all inquiry calls related to patient referrals related to pharmacy services including but not limited to follow up for admitted patients. Coordinates ongoing clinical assessments for new and existing patients where appropriate.Supports supervisor as team lead and generates reports as needed for operational metrics.

Education and Certification Requirements:

High School Diploma or Equivalent (Required)Advanced Certified Pharmacy Technician (ACPHT) - Pharmacy Technician Certification Board (PTCB), Certified Pharmacy Technician Exam (EXCPT) - National Healthcareer Association (NHA), Registered Pharmacy Technician (RPT LICENSE) - State of Florida (FL)

Additional Job Information:

Complexity of Work: Requires critical thinking skills, effective communication skills, knowledge of medical terminology, decisive judgment, good computer skills and the ability to build and foster positive relationships. Must be able to work in a stressful environment and take appropriate action. Required Work Experience: Two (2) years of infusion and/or specialty pharmacy experience. Other Information: Additional Credential Info: Certified pharmacy technician through a nationally accredited organization
