Support patients and teammates in discovering new and sustainable ways to improve their health. As an Oncology Health Coach I, you’ll build meaningful coaching relationships, help individuals unlock their personal motivation for lasting behavior change, and guide them through the resources and programs available to support their well‑being. This hybrid role blends motivational coaching, clinical collaboration, patient navigation, and education—all aimed at improving long‑term health outcomes and enhancing the overall patient/teammate experience.

Essential Functions

  • Provide individualized one‑on‑one health coaching sessions (30–60 minutes, based on participant needs) focused on participant‑identified goals and sustained behavior change.
  • Deliver health coaching in alignment with NBHWC guidelines, utilizing evidence‑based approaches including motivational interviewing, behavior change strategies, goal setting, and barrier assessment/problem solving.
  • Collaborate with patients and teammates, including medically vulnerable or chronically ill populations, to support active engagement in care through education, encouragement, and self‑directed goal achievement.
  • Develop and monitor individualized well‑being and care plans; support shared decision‑making and communicate pertinent information with the clinical care team and other involved providers.
  • Provide patient navigation, advocacy, and referrals to assistance programs, mental health resources, and community‑based services.
  • Deliver customized, evidence‑based education in areas such as physical activity, weight management, tobacco cessation, stress reduction, and chronic disease self‑management, while remaining within scope of practice.
  • Communicate with patients, caregivers, and teammates via telephone, electronic platforms, in‑person visits, and group sessions.
  • Review relevant patient information prior to sessions and document coaching encounters accurately in the electronic health record and/or well‑being platform in accordance with organizational policies, confidentiality standards, and applicable law.
  • Support condition‑specific initiatives, data collection, and program evaluation efforts in collaboration with key stakeholders.
  • Maintain an active NBHWC‑recognized health coach certification and comply with all professional and organizational standards.

Education, Experience & Certifications

  • Bachelor’s degree required; degree in a health‑related field (e.g., health promotion, health education, nursing, athletic training, nutrition, counseling, biology, or related discipline) preferred.
  • Minimum of two years of professional health coaching experience required.
  • Current health coach certification from an NBHWC‑approved training program required.
  • Demonstrated practical skills in evidence‑based behavior change methodologies.
  • Experience using healthcare technology platforms preferred.
  • Prior experience providing individual (1:1) and group coaching sessions preferred.
  • Experience working collaboratively as part of an interdisciplinary care team preferred.
  • Demonstrated commitment to delivering high‑quality, patient‑centered care.
  • Comfortable presenting and facilitating sessions both virtually and in person, including with larger groups.
