The RN Clinical Assistant is responsible for facilitating the efficiency and workflow of the Hospitalist Service . The RN Clinical Assistant acts as a liaison between the Hospitalist providers and other members of the health care team. The RN Clinical Assistant works with other members of the health care team to facilitate discharge planning and ensure a smooth transition of care back to the primary care provider. The RN is accountable to practice in accordance with the Michigan Public Health Code Act 368 of 1978, Article 15 (Health Occupations), Part 172 (Nursing). The RN must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.

Pay Range:  $31.23 - $46.84/hour

Employment Type:  Part Time, 48 hours every 2 weeks

Schedule:  8:00 am - 4:30 pm on varied weekdays, includes every 3rd weekend

Requirements:  Graduate of accredited Registered Nursing Program required, Current Michigan RN License required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required

Standards of Professional Practice

  • Promotes patient safety by maintaining the policies and procedures of the hospital and nursing department, including maintaining the Patient's Bill of Rights.

Facilitation/Coordination of Workflow

  • Reviews assignment for potential discharges.
  • Reviews patient's chart to develop a general awareness of patient's needs.(vitals/lab/prior day's test results).
  • Communicates with physician and advance practice providers on the status of patient's discharge care plan.
  • Triages calls from the nursing unit, care management and other members of the patient's care team and refers them to provider as indicated.
  • Keeps the assigned unit nurse informed regarding the plan of care.
  • Initiates paperwork/processes needed to facilitate patient discharge

Discharge Planning

  • Participates in care coordination with the Care Manager and other members of the health care team.
  • Facilitates bed availability by keeping physicians informed regarding the location of priority discharges.
  • Prepares discharge instructions for patients prior to discharge, including those related to medications, lab draws and various testing.
  • Contacts primary care provider at the time of discharge as needed to make them aware of patient's immediate post discharge follow up needs.
  • When applicable, schedules patient's post hospitalization follow up appointment.
  • Establishes a one time follow up appointment for patients without a PCP.
  • Ensures discharge paperwork is faxed to necessary members of the care team
