Meet face-to-face with all patients to assist in the facilitation of recovery services and ensure that all recovery activities are properly documented in the patient’s record
Utilize collateral contacts and other local referral agencies to provide services not offered by the program
Conduct monthly Care Coordination meetings with patients, and complete corresponding documentation, to assure that patients are engaged with the services they need
Working in collaboration with the buprenorphine-waivered practitioner who is prescribing buprenorphine or naltrexone products to individuals with moderate to severe Opioid Use Disorder
Perform other duties as assigned by the Operations Director of OBOT Services
A bachelor’s degree in social work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and at least either one year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.
Licensure as a Registered Nurse with at least either one year of direct experience providing services to individuals diagnosed with a substance use disorder or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.
Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115-50-10
Monday-Friday; 8:00am-4:00pm
