Role Description

The Lead Director, Care Management collaborates to drive care management transformation as subject matter expert for integrated care management models. This leader collaborates in vision setting and is accountable for effective design of evidence-based care management workflows, and drives alignment across nursing care management, social work, and community health in value-based care. As a critical cross-functional partner to Population Health and other business functions, the Lead Director operates to ensure a cohesive care management model implementation; driving optimization and innovation.

Responsibilities

Strategic Planning 

  • Serve as an organizational subject matter expert for care management, bridging knowledge from diverse models and external benchmarks to inform Oak Street Health’s strategic direction.

  • Validate care management approaches through data analysis, literature review, and outcome measurement; drive adoption of proven, scalable solutions.

  • Serve as a critical partner to Population Health and other cross-functional teams, fostering collaboration and shared accountability for patient outcomes

  • Mentor and develop care management leaders and team members on evidence-based practices, change management, and integrated care delivery.

  • Represent CVS Health in enterprise-wide initiatives, industry forums, and external partnerships related to care management innovation.

Operational management 

  • Collaborate on the design, implementation, and continuous improvement of clinical and non-clinical care management workflows, ensuring alignment with evidence-based practice and strong operational rigor.

  • Co-lead (dyad) care management initiatives alongside operational leaders to ensure seamless integration of nursing, social work, and community health/care navigation functions.

  • Identify and address gaps in care model cohesion, workflow efficiency, and patient experience by engaging clinical and non-clinical stakeholders.

  • Other duties as assigned.

  • A strategic thinker with strong analytical, problem solving, group facilitation, presentation skills 

  • Ability to exceed in a fast-paced environment while sifting through details quickly and staying focused on goals and deliverables 

  • Outstanding written and verbal communication and conflict resolution skills

Minimum Requirements: 

  • 8+ years of progressive leadership experience in scaled care management models required, experience in VBC and population health models strongly preferred.

  • Advanced degree required (MSN, MPH, MHA, DNP or equivalent).

  • RN licensure strongly preferred 

  • 10 or more years of healthcare experience working as an RN, preferably in a primary care office or other outpatient setting, preferred 

  • Demonstrated expertise in workflow design, program implementation, and evaluation of evidence-based care management approaches.

  • Strong cross-functional leadership skills; experience influencing and partnering across nursing, social work, and community health.

  • Proven ability to lead large-scale, complex initiatives with measurable impact on outcomes.

  • Excellent communication, change management, and stakeholder engagement skills.

Pay Range

The typical pay range for this role is:

$100,000.00 - $231,540.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.
