Position Summary
This work‑from‑home role is responsible for reviewing and triaging Care Team tasks, identifying key patient information, and supporting care coordination processes. The position screens members for needed services using established guidelines, makes referrals to clinical staff, and monitors cases for accurate updates. The role also supports care plan development by arranging services under clinical direction, promoting communication across care teams, and conducting non‑medical research to maintain and close cases.

The Care Coordinator accurately documents required information, assists with resolving issues, protects member confidentiality, and ensures compliance with regulatory and accreditation standards. Strong communication, customer service, organization, and computer skills are essential, along with the ability to navigate multiple systems and work effectively within a multidisciplinary team.


Required Qualifications

  • Must Reside in the State of Texas
  • 2+ years of experience in healthcare support, customer service, call center, medical office, or related administrative role
  • Experience working with patients, members, or customers in a service‑oriented environment
  • Strong communication skills (verbal, written, and telephonic)
  • Excellent organizational and time‑management skills
  • Ability to multitask and manage multiple cases or tasks simultaneously
  • Basic understanding of medical terminology and general healthcare concepts
  • Strong customer service focus, with the ability to handle sensitive situations and promote positive member experiences
  • Critical thinking and problem‑solving ability

  • Ability to work effectively in a remote, structured environment
  • Ability to collaborate with a multidisciplinary team
  • Commitment to maintaining confidentiality and complying with regulatory and company standards
  • Strong ability to follow workflows, guidelines, and established processes
  • Proficiency using computers and navigating multiple systems
  • Comfort with Microsoft Office tools, especially Word and Excel
  • Ability to learn and work within internal case management platforms and databases

  • High School Diploma or G.E.D.

Pay Range

The typical pay range for this role is:

$18.50 - $35.29

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
