Job Description

Post Acute Care Units

A Wing is a 58 bed Medicare/Medicaid unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring.  Nurses work closely with a variety of health care professionals including: doctors, social work, dieticians, PT/OT, hospice, and spiritual support.  A Wing offers a well-rounded opportunity for learning, professional growth and personal satisfaction.

B Wing is a 40 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses.  B wing is a great unit for nursing students to develop their organizational skills and gain some experience in dealing with challenging behaviors. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care. 

C Wing is a 58 bed Medicare/Rehabilitation and Skilled unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals.  

About the facility 

With 2 floors, 4 units, we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and a rehab residents. With 196 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate in

multiple activities.

Scope of work 

Licensed Practical Nurse (LPN)

Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines. 

Qualifications

  • Required Graduate of an approved LPN program.
  • Experience in Skilled Nursing/Long Term Care. Preferred
  • LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required
