As an Advanced Care Patient Liaison, you will play a vital role in supporting patients and families through education, communication and compassionate guidance regarding Hospice services. In collaboration with providers, you will connect with patients and families to share information about Hospice, answer questions and provide ongoing education at a pace that respects their readiness to consider services. This position serves as a trusted resource, maintaining regular touchpoints with families, facilitating smooth transitions when patients are ready, and acting as a liaison between medical teams, home care and Hospice services. The role also includes developing and strengthening relationships with skilled nursing facilities and other care partners across Northeast Ohio. While travel to local facilities is required, there is flexibility to work remotely when not on-site.

A caregiver in this role will work Monday through Friday from 8:00am to 5:00pm with no weekend or holiday requirements.

A caregiver who excels in this role will:

  • Work with providers regarding communication with patients and provide hospice education to patients/families if necessary.
  • Continually provide regular touch points and ongoing education if patient/family is not willing to consider hospice services.
  • Review list of Cleveland Clinic patients with scores that place patient into high risk category for life limitation and conduct a chart review of identified patients and provide information to providers on suggested levels of care.
  • Assist in developing plans of care for patients/families.
  • Server as the resource person to provide psychoeducation, discussion of options, financial concerns and recommendations to engage the patient/family to help reduce social, psychological, financial and behavior barriers to successful medical outcomes.

Minimum qualifications for the ideal future caregiver include:

  • Master’s Degree in Social Work from an accredited school of social work (CSWE) OR a graduate of an accredited school of professional nursing
  • Current licensure as a social worker in the State of Ohio OR current licensure as a Registered Nurse (RN) in Ohio
  • Minimum of one year work experience as a Master’s prepared Social Worker OR minimum one year of current work experience as an RN in an acute care setting or SNF

Preferred qualifications for the ideal future caregiver include:

  • BSN
  • LISW licensure
  • Prior experience caring for hospice patients
  • Prior experience in oncology

Physical Requirements:

  • Manual dexterity to operate office equipment.

  • Requires good visual acuity through normal or corrected vision. Must be able to hear normal conversation.

  • Requires sitting for extensive periods of time.

  • Light Work
