Job Description Summary:

Successfully fulfills the criteria and experience profile for Registered Nurse, in addition to the responsibilities outlined in this job description. The Pharmacy Intake RN demonstrates technical, time management, communication, coordination, clinical excellence, and leadership skills necessary to support business needs regarding Infusion Pharmacy referrals. This position ensures physician orders are in place, performs ICD-10 coding, processes referral information, reviews drug qualifications, and participates in discharge planning of patients. Maintains proficiency through process improvement and operational efficiency. Participates in mutual communication with departmental and interprofessional teams to ensure smooth transition of care. This position will be located on-site at the home infusion pharmacy.
