What you will need:
Minimum Formal Education Required: Graduate from a school of Physical Therapy with a Bachelor of Science or master's degree from an accredited college or university approved by the APTA and the Council on Medical Education of the AMA.
License, Certification, Registration Required: Must be licensed or eligible for licensure as a Physical Therapist in the State of IL; CPR certification.
Minimum Length & Nature of Experience Required: No clinical experience required; entry level position.
Special Knowledge and Skills Required Must demonstrate good interpersonal and communication skills in dealing with physicians, other health professionals, patients, family members, and the community. Must demonstrate good organizational and time management skills. Must be able to work effectively in an independent environment. EPIC experience preferred.
What you will do:
The role of the physical therapist is to support the Healthcare Transformation Program’s physicians overall and collaborate with the program’s sports medicine specialist. Through the Healthcare Transformation Program, Swedish Hospital and NorthShore University HealthSystem Medical Groups are embedding specialty care (cardiology, sports medicine/orthopedics, endocrinology, pulmonology, dermatology, and gastroenterology) into Federally Qualified Health Centers (FQHCs) to increase health access and equity. The role provides professional physical therapy evaluation, treatment and follow-up services to physician-referred patients. The physical therapist will be responsible for assessing patients and performing procedures consistent with or based on the age and developmental status of the patient to maximize the clinical outcome and safety of the patient.

Responsibility/ Duties:
1. Corporate Responsibility
Knowledge of federal, state and local laws that apply to job and area of responsibility.
Knowledge and application of NUHS policies, procedures and service standards that apply to job and area of responsibility
Ongoing growth and development from participation in events such as workshops, online trainings, in-service programs and departmental meetings.
Willingness to travel regularly to FQHC sites, all within a 5-mile radius of Swedish Hospital.
Provide and assist with patient care at FQHC locations in addition to Swedish Hospital.
3. Documentation
Prepares and maintains accurate patient records including evaluations, progress notes, discharge summaries and billing as required by Medicare, third party payers, and JCAHO/HFAP. Assures co-signature on notes and evaluations if waiting on licensure exam results.
Issues written home exercise programs.
Participates in peer review process of chart audits.
4. Communication
Reports on patient/resident progress at staffing and other conferences.
Attends patient care/resident staffing, family conferences, and hospital rounds as appropriate.
Confers with supervisory therapists, other staff therapists, physicians and other related personnel regarding progress of patient/resident as needed.
Communicates and collaborates with physician and other health professionals and family or caregivers to facilitate optimal patient/resident care.
