Job Summary: The Virtual Health Coordinator serves as a resource to promote excellence in virtual patient care. This individual assists in the development, implementation and ongoing performance management of the Virtual Health program throughout Wellstar. This position serves as a clinical role model, educator, and liaison for the Virtual Health program, hospital staff and community. This position also collaborates with the physician champions/leaders and Service Line leadership to facilitate and promote improved outcomes. This position reports directly to the AVP Virtual Health. This leader will be responsible for program goals and operations regarding all aspects of the Virtual Health program. 1) Provides direction to establish, monitor and evaluate the quality of care given to virtual health patients and to ensure our patients are provided with the highest quality of care. An additional focus of this position is to provide vision and ensure the program stays attentive to current and future community needs as well as national trends. Examples of this would include routine literature review, best practice sharing, and/or attending pertinent conferences. 2) Assumes responsibility for implementing standards of practice, program policies and procedures and maintaining and improving the quality of patient care delivered. 3) Creates an environment which enables the system to fulfill its mission and goals, fosters communication and a collaborative practice with physician and other departments. The Virtual Health Coordinator assumes responsibility for data collection and reporting, performance improvement, staff education, and program coordination for protocols. This individual works collaboratively with leaders at the hospital and system level to establish and implement standards for Virtual Health patient care with the goal of improving patient care and clinical outcomes. Prepare management reports associated with the performance of the program. Present data and reports to the Service Line leadership, Nursing leadership and hospital departments. Core Responsibilities and Essential Functions: Assumes responsibility for the ongoing coordination of virtual health for sub-specialties across the continuum from admission to discharge. - Collaborates with the Coordinator Teleneuroscience for the stroke program to ensure alignment with processes. - Facilitates collaboration related to clinical protocols/order sets and care paths. - Analyzes, interprets and makes recommendations based on clinical outcome data. - Monitors adherence to clinical protocols, tracks variances from protocols/care paths and reports outcome data to nursing and sub-specialty physician leaders. - Ensures compliance with The Joint Commission standards. - Meets regulary with sub-specialty program Managers to assure cohesive integration of virtual health. - Manage each initiative using project management skills. Serves as a clinical expert for complex and/or highly specialized patient care situations to hospital staff/personnel and other health care professionals within the organization and community. - Demonstrates, via role modeling and consultation, advanced knowledge and practice in the assessment, diagnosis, treatment and evaluation of human responses to actual and/or potential health problems. - Consults with healthcare providers, ancillary services, patients and significant others to assess and identify individual patient care needs. Promotes and develops personal and professional growth and development of self and patient care staff. - Provides resources and tools for staff as needed to support the development. - Participates in identifying learning needs, planning and implementing educational programs on an annual basis for those areas involved in stroke care. - Incorporates appropriate teaching and learning principles into the design and delivery of educational programs and materials. - Demonstrates motivation for learning through independent reading, professional networking and communicates professional expertise through publications and presentations at the local, regional, and national level. Establishes and fosters collaborative working relationships with physicians involved in the care of the patient. Provides leadership in facilitating appropriate communication systems between the physicians and the other health care professionals. Utilizes effective verbal and written communication skills and maintains appropriate interpersonal relationships. - Fosters a cooperative spirit among team members. - Utilizes appropriate intervention behaviors and communication styles in order to be an effective liaison and resource person. - Maintains a climate of open communication by listening, providing regular feedback, and involving employees in process implementation. Required Minimum Education: Graduate from an accredited school of nursing Required and Bachelor's Degree Science Required and Master's Degree Preferred Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.

Additional License(s) and Certification(s): Required Minimum Experience: Minimum 3 years acute care clinical experience Required and experience in healthcare quality and performance improvement. Preferred Required Minimum Skills: Must have excellent communication and presentation skills both orally and written. Should be highly intelligent, motivated with sense of urgency, professional, and polished. Must have the ability to inspire trust and credibility among senior leaders, physicians, and physician staff. Must have the ability to critically think, possess exceptional interpersonal skills and business savvy. Demonstrated ability as an individual contributor, while exceling as a team player and good at mobilization of people. Must have proficient knowledge of Microsoft Office (Word, Excel and PowerPoint). Must be organized and attentive to detail. Exceptional customer service skills. Must be able to handle multiple high priority tasks. Must be able to manage several different projects at the same time. Must be able to operate a car and travel to all facilities and hospitals when needed. Must be able to work independently and use good judgement according to established policy. Ability to analyze opportunities and threats, design strategies to grow the program, communicate effectively and build relationships. Ability to work in a rapidly changing and dynamic environment.
