As the Veteran Care Coordination Specialist, you’ll contribute to our success in the following ways:

  • Managing veteran referrals from intake through care coordination and service initiation
  • Ensuring referrals are accepted, processed, and documented accurately and timely
  • Entering and tracking all accepted and denied referrals within required systems
  • Building patient profiles and supporting onboarding activities
  • Coordinating care services and initiating care plans as appropriate
  • Serving as a key point of contact for veterans, family members, VA representatives, and internal teams
  • Providing timely communication and updates regarding referral status and care coordination activities
  • Monitoring expiring authorizations and proactively working to prevent service interruptions
  • Identifying and resolving barriers to care to improve service delivery and reduce unnecessary discharges
  • Maintaining accurate documentation and compliance within all required systems
  • Supporting operational excellence through attention to detail, organization, and follow-through
  • Ensuring all activities align with the vision of Elara Caring's leadership team and VetCare program
  • Promoting Elara Caring's mission, values, and commitment to quality care
  • Maintaining patient and employee privacy and confidentiality in accordance with HIPAA regulations
  • Performing other duties and special projects as assigned
     

What is Required?
 

  • High School Diploma or GED required
  • Associate's or Bachelor's degree in Healthcare Administration, Business, or a related field preferred
  • Minimum of one (1) to two (2) years of experience in healthcare, home care, case management, care coordination, or a related field
  • Experience working with VA referrals, veterans' services, or healthcare coordination preferred
  • Strong written and verbal communication skills
  • Exceptional organizational and time management abilities
  • Ability to manage multiple priorities and high-volume referral activity
  • Strong data entry and computer skills
  • Ability to work independently and collaboratively within a team environment
  • Excellent problem-solving skills and attention to detail
     

You will report to Care Team Manager, Lead, Branch Director, or Regional Branch Director.
