Job Title: Registered Nurse (RN) Case Manager – Hospice Care
Location: Lenawee County
Schedule: Monday through Friday, 8:00a-5:00p

Position Overview: As a Registered Nurse (RN) Case Manager in our Hospice service line, you will provide compassionate, high-quality care to patients in their homes, helping them navigate the challenges of serious illness. You will work collaboratively with a multidisciplinary team to ensure that each patient receives personalized care that aligns with their unique needs and wishes. This role offers a unique opportunity to build lasting relationships with patients and their families while delivering care that truly makes a difference.
Key Responsibilities:
Manage and coordinate care for a caseload of hospice patients, ensuring that care plans are developed, implemented, and updated according to patient needs.
Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
Develop and revise individualized care plans in collaboration with the patient, family, and interdisciplinary team.
Provide direct nursing care, including symptom management, pain control, and emotional support, to hospice patients in their homes.
Educate patients and families on the hospice philosophy, goals of care, and the dying process.
Serve as a liaison between patients, families, and healthcare providers to facilitate communication and coordinate care.
Monitor and document patients' progress, adjusting care plans as necessary to meet changing needs.
Participate in interdisciplinary team meetings to discuss and evaluate patient care plans.
Ensure compliance with all federal, state, and local regulations, as well as company policies and procedures.
Provide on-call support as needed to ensure that patients receive timely care and support.
Qualifications:
Current and unrestricted Registered Nurse (RN) license in the state of Michigan.
Minimum of 1-2 years of nursing experience, preferably in hospice, palliative care, or home health.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients and families.
Compassionate, empathetic, and dedicated to providing high-quality care to patients with serious illness.
Ability to work independently and as part of a multidisciplinary team.
Valid driver’s license and reliable transportation to travel within the designated coverage area.
