Embark on a deeply rewarding journey as a Disability Support Practitioner within the Lifestyles Home and Community team, and experience the fulfillment of helping clients overcome challenges, achieve their daily goals, and thrive in a nurturing and supportive environment.

Primarily, this position includes home and community-based one-on-one support where you'll have the opportunity to directly engage with mature aged clients (aged 18 - 80) and encourage independence by taking them out into the community.

The Role

This role is based across the North Shore and Ryde region, requiring travel to our group homes in Denistone, Epping, Ryde, West Ryde, Hornsby, Pennant Hills, West Pennant Hills, Wahroonga, Marsfield, and Normanhurst. The position is permanent part time (18 hours per week) and offers day shifts from 9:00 AM to 3:00 PM. Must be available to work Monday - Wednesday.

  • Delivering personalised support by assisting clients at mealtimes, personal care, toileting, administrating medication and manual handling
  • Professional Behaviour Support experience
  • NSW Driver's Licence (P2 or above)
  • Physical capability to support clients with mobility and transfers
  • Available from 9am - 3pm Monday - Wednesday
  • Comfortable travelling across the North Shore and Ryde region
