As a Nurse Practice Manager, you will oversee the intake process for surgical candidates by obtaining clinical information in advance related to each patient’s health status and level of functioning. In this role, you will work collaboratively with the multidisciplinary care team to coordinate care and manage disease longitudinally for patients requiring Cardiothoracic Surgery, with a focus on patient triage prior to surgery and ensuring all pre-operative testing is completed to allow surgeons to function at the top of their license. Additionally, you will provide pre-operative patient education, assist with post-operative clinic visits and coordinate appointment scheduling. This role is key in managing transitions of care for cardiothoracic patients, ensuring effective communication, identifying barriers, facilitating solutions and optimizing patients for open-heart or laparoscopic heart surgeries.

A caregiver in this role works days from 8:00 a.m. – 5:00 p.m.

This is a hybrid position following the new hire period, allowing the option to work from home one to two days per week.

A caregiver who excels in this role will:

  • Manage the intake process for surgical candidates by obtaining, in advance, clinical information relative to the patient's state of health and level of functioning. 

  • Ensure departmental compliance with regulatory and The Joint Commission requirements. 

  • Responsible for coordination and continuity of care working with the PCP's, referring physicians, patients, CCF cardiology staff, staff surgeons and their office staff, other nurse practice managers (NPM), as well as scheduling officers, preop/TCI/postop nurse clinicians, to assure continuity of care and appropriate concern for individualized pre-surgical and/or postsurgical needs. 

  • Review all clinic information available for review with surgeon for appropriate pre-surgical test and appointment scheduling.

  • Communicate additional studies to the outpatient scheduling office under the physician's direction. 

  • Facilitate hospital transfers in collaboration with scheduling officers, access plus and surgeons' office. 

Minimum qualifications for the ideal future caregiver include:

  • Bachelor's of Science in Nursing (BSN) from an accredited School of Nursing 

  • Current state licensure as a Registered Nurse (RN) 

  • Basic Life Support (BLS) Certification through American Heart Association (AHA) 

  • Four years of experience as a nurse in medical cardiology or cardiac surgery clinical areas 

Preferred qualifications for the ideal future caregiver include: 

  • Ability to work with multiple disciplines at all levels

  • Ambulatory or care coordinator background

Physical Requirements:

  • Requires the ability to walk to various locations throughout the foundation, normal or corrected vision and hearing to normal range and the ability to work and function in a stressful environment.

  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

  • Physical Demand requirements are in excess of those for Light Work.
