SUMMARY

Consistently functions as clinical resource and professional leader for Orthopedics with focus on Total Joint Replacement patients.

RESPONSIBILITIES

  • Develops, implements, and evaluates programs to enhance the care of the orthopedic patients.

  • Acts as a role model, promotes RRH philosophy of Nursing excellence through guidance, precepting, leadership, teaching and practice.

  • Communicates and applies specialized knowledge and expertise to meet individual patient and family needs.

  • Identifies and works to eliminate/minimize system variations and service delivery barriers that prevent achieving optimal patient outcomes in a reasonable timeframe with cost effective use of resources.

  • Identifies, participates in, and implements projects for patient teaching and improving patient care; including community outreach programs.

  • Daily rounding to monitor patient progress and outcome attainment in acute, and rehabilitation care settings.

  • Establishes and maintains mechanisms for communicating and collaborating with the multidisciplinary team, involving all staff in sharing/gathering pertinent information and in problem solving.

  • In concert with multidiscipline team, identifies across continuum of care needs and works to eliminate obstacles to a safe discharge and achievement/maintenance of health.

  • Home Care referrals (out of country).

  • Involvement in patients transferred to TCU from other facilities.

  • Tours of patients and families interested in utilizing TCU as rehab facility.

  • Attends daily discharge rounds to assess/prioritize patients awaiting transfer to TCU.

  • Routing interfacing with physician office staff to provide periodic program updates.

REQUIRED QUALIFICATIONS

  • Diploma or Associate’s Degree in Nursing required.

  • Registered in NYS as a Registered Professional Nurse.

PREFERRED QUALIFICATIONS

  • Bachelor’s Degree in Nursing preferred.

  • Orthopedic Nurse certification preferred.

  • 5 years of acute hospital care experience preferred.

  • 4-5 years’ experience in a orthopedic or neurosciences setting preferred.

  • 1 year supervisory or program coordination experience preferred.

  • Excellent oral and written communication and interpersonal skills.

  • Requires problem solving, decision making and critical thinking skills.

  • Ability to recognize and manage emergencies.

  • Strong knowledge of specific chronic care regimen.

  • Time management and organizational skills.

  • Must be able to work in a self-directed environment, with ability to work with and lead teams.

  • Patient advocate.

$80,000.00 - $110,000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
