$10,000 Sign-On Bonus

Schedule: Monday - Friday, 8:00 AM - 5:00 PM; with on-call requirements

Location: Bloomington, IN and surrounding counties

As an RN, you will have the opportunity to provide one‑on‑one patient care and practice at the top of your license. You will utilize your leadership skills to coordinate care and provide home‑based nursing services as directed by the attending physician. You’ll be part of an interdisciplinary team focused on compassionate, high‑quality care and positive patient outcomes.

You’ll also interact with families, supporting them throughout the care journey — one of the most rewarding parts of home care. Home health allows you to see the real‑world environment influencing your patients’ recovery and create meaningful, personalized care plans.

HOW YOU'LL WORK:
You’ll provide skilled nursing care in the patient’s home as directed by the attending physician. This role offers autonomy, so time management and organization are essential. You will be responsible for duties within the full scope of practice as outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:

Plan of Care:
Observe and monitor patient conditions and perform OASIS assessments (training provided if needed) to create and adjust individualized care plans.

Minimize Patient Risk:
Administer medications as prescribed and reduce re‑hospitalizations by prioritizing high‑risk patient visits.

Continuity of Care:
Coordinate multidisciplinary care and ensure proper admissions, transfers, and discharges.

Family Educator/Advocate:
Educate patients and families on disease processes using teach‑back methods to ensure understanding.

Policies:
Complete clinical documentation per agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care.

Rules and Regulations:
Understand and follow agency policies, procedures, and regulations. Communicate schedule or availability changes promptly.

Operations:
Attend in‑service training sessions and mandatory agency meetings.

REQUIREMENTS:

  • Registered Nurse with a current state license
  • Minimum one year of RN experience in an acute care setting
  • Home care experience preferred
  • Valid driver’s license and auto insurance in your name
  • Capable of meeting all physical demands of the role
