Field Nurse Practitioner - Nevada

*Candidate must reside And be licensed in the state of Nevada.

Location: This Field Based position will be visiting patients/members in assisted living facilities, memory care, group homes and skilled nursing facilities in Henderson and South Las Vegas, Nevada.

**Eligible for mileage reimbursement.

Required Work Shift: 5 days a week, Monday thru Friday, 40 hours a week.

The Field Nurse Practitioner is responsible for managing patient care and treatment (in person, telephonically and/or video). 

How will you make an impact: 

  • Conducts daily visits with members/patients that reside in either assisted living facilities, memory care, group homes and skilled nursing facilities.

  • Provides a range of primary, acute and/or specialty healthcare services for patients at specific facilities, telephonic or virtually. Adjusts to specifics.

  • Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. 

  • Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices. 

  • Will work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team. 

  • Formulates and documents care plan. 

  • Prescribes medication or other forms of treatment. 

  • Participates in On-Call program, which may include after-hours, weekend and holiday calls and visitations. 

Minimum Requirements:

  • Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. 

  • Current, unrestricted RN license and NP license in the state of Nevada is required. 

  • Possession of DEA registration or eligibility required.

  • For Carelon Health business units, satisfactory completion of a Tuberculosis test is a requirement for this position.

Preferred Skills, Capabilities and Experiences:

  • Experience working with the geriatric patient population.

  • Home health and/or hospice experience.

For candidates working in person or remotely in the below location(s), the salary* range for this specific position is $110,880 to $166,320.
