Key Position Details:

Population Health Pharmacy Liaison

A population health pharmacy liaison is a key member of the population health team who supports the delivery of high-quality, patient-centered care. The population health pharmacy liaison works with a pharmacist and collaborates with other health care providers to improve medication adherence and health outcomes, and reduce health care costs for a defined population of patients.

  • Provides patient outreach to improve medication adherence and health outcomes.
  • Coordinates with providers and pharmacy to overcome formulary and prior authorization issues that occur.
  • Educate patients on formulary and insurance coverage of medications.
  • Assists staff and patients with available manufacturer coupon and prescription assistance programs.
  • Assists in problem solving potential issues caused by the patient’s inability to afford the co-pay.
  • Hand off medication therapy problems and clinical concerns to the population health pharmacist.
  • Document communications in the electronic medical record.
  • Participate in quality improvement initiatives and data analysis to evaluate the impact of population health pharmacy services.
  • Attend and contribute to team meetings, trainings, and continuing education activities.
  • Other duties as assigned.

Hybrid position - some of the initial training will be onsite at the Commons, in Minneapolis, MN

Based on business needs, there will also be occasional onsite meetings

Job Description:

Increases capture rate and improves patient care through a focus on eliminating obstacles during the discharge process that might prevent the patient from leaving the hospital with their medications in hand. Reviews upcoming discharges to ensure patient’s prescription benefit information is correct and intended discharge medications are covered. Coordinates with hospital and pharmacy staff to obtain prior authorizations, medication patient assistance enrollment, and acts as a first contact point to resolve formulary issues.

Principle Responsibilities

  • Preforms discharge preparation.
    • Reviews upcoming discharges to ensure that the patient’s prescription benefit information is in the electronic medical record system.
    • Reviews upcoming discharges to identify patients that might be discharging on potential problem medications.
    • Runs test claims to verify formulary status and copay of potential problem medications.
  • Coordinates prior authorizations and formulary overrides.
    • Assists in expediting prior authorizations.
    • Coordinates with providers and pharmacy to overcome formulary issues that occur upon discharge.
  • Provides prescription assistance.
    • Assists staff and patients with available manufacturer coupon and prescription assistance programs.
    • Assists in problem solving potential issues caused by the patient’s inability to afford the co-pay.
  • Provides patient outreach.
    • Educates patients and hospital staff on medication options.
    • Encourages patients and hospital staff to utilize hospital pharmacy services.
  • Other duties as assigned.


Required Qualifications

  • Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
  • Some experience as a Pharmacy Technician


Preferred Qualifications

  • High school diploma or GED
  • Vocational or Technical Training from an American Society of Health-System Pharmacists (ASHP) accredited pharmacy technician training program


Licenses/Certifications

  • Registered Pharmacy Technician - MN Board of Pharmacy required upon hire
  • Certified Pharmacy Technician - Pharmacy Technician Certification Board required within 180 Days
  • For Population Health Liaison – no registration or certification needed
