Job Description Summary:

This position coordinates the rehabilitation of the post cardiac event, CABG, and high-risk patients. Implements secondary prevention strategies to include exercise and patient education and risk reduction. Provides rehabilitative services for post cardiac, pulmonary, and patients with multiple risk factors.

Qualifications:

  • Candidate must meet a minimum of one (1) of the following educational requirements:
    • MS in physical therapy
    • Respiratory therapy degree with certification
    • ACSM Registered Clinical Exercise Specialist certification
  • Current HealthCare Provider CPR & AED required.
  • Current ACLS certification preferred or must successfully obtain within one (1) year of hire.
  • Certification in personal training, health and wellness coaching, and/or group fitness instruction preferred.
  • Minimum of five (5) years’ experience with chronic disease population; delivering personal training, health and wellness coaching, and/or group fitness instruction preferred
  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
  • Commitment to working in a team environment and maintaining confidentiality as needed.
  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Coordinates patient referrals by contacting the patient, researches insurance coverage details, per department procedure and protocol.
  • Performs cross-referrals to appropriate health/wellness programs to include, but may not limited to; smoking cessation, diabetes education, heart failure clinic, etc.
  • Utilizes American Association of cardiopulmonary rehabilitations standard for appropriately screening new patients and as set forth by the cardiopulmonary rehabilitation policies and procedures. 
  • Communicates with the referring physician regarding the need for a referral and provides recommendations for patients that are not eligible for cardiopulmonary rehabilitation reimbursement.
  • Conducts appropriate pre-exercise assessment of participants per department procedure and protocol. Creates and updates individualized treatment plans in a timely manner.
  • Utilizes appropriate protocols to evaluate patient functional capacity and develops individualized exercise programs.
  • Provides educational program for patient per department protocol.
  • Compiles program census and utilizes accurate billing codes and procedures to charge patients.

Schedule:

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
