🕒 Schedule: Monday–Friday | 8:00 AM – 5:00 PM
📍 Territory: Lansing & Surrounding Communities | Field-Based | Full-Time

🌿 In This Role, You Will:

• Provide direct, in-home nursing care to hospice patients

• Create and manage individualized care plans based on patient needs

• Monitor symptoms and communicate changes proactively

• Collaborate with physicians, social workers, chaplains, and other team members

• Guide and support families through education and emotional care

• Deliver dignified, person-centered end-of-life care

💼 What You Bring:

• Current RN license in the State of Michigan

• 1+ year of nursing experience (hospice or home health preferred)

• Strong clinical judgment with a heart for holistic care

• Solid communication skills and emotional resilience

• Ability to lift up to 50 lbs and travel locally within the Lansing area
