JOB DESCRIPTION:

SUMMARY OF POSITION

Responsible for providing wrap-around and/or intensive care coordination to youth and families with high needs. This includes in-person, community based child and family team meetings, IMCANS review, crisis services, prevention and safety plan review. Works collaboratively with other service teams to meet the needs of clients assigned via Pathways. May be responsible for assisting with linking clients to appropriate services, advocating on their behalf, providing support and monitoring effectiveness of services as needed.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides direct client services. Manages case load of life skills training for clients.
  • Facilitates client’s use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties.
  • Assesses and develops person-centered plan and monitor progress of the plan.
  • Collaborates with team and outside agencies. Serves as the main point of connect for communication with resources or other entities to ensure continuum of care.
  • Completes client assessments (ex: Ansa, PHQ9, GAD7) and develop care plans, monitor client’s progress through care plan.

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Customer service skills.
  • Time management and organization skills.
  • Analytical thinking skills.
  • Therapeutic skills and knowledge of EBPs.

QUALIFICATIONS

Education Level              

Bachelor’s degree from a four-year college or university; preferably in psychology, social work, or a related field.

Years of Experience       

2 years of experience is preferred.

$16.50--$23.10
