JOB DESCRIPTION:

SUMMARY OF POSITION

Works with enrolled parents to assist them in creating and building connections in order to assist them to become self-sufficient. Provides case management and outreach services. Assesses clients to identify their family strengths, challenges, and needs. Collaborates with community partners to build connections. Assists and facilitates parent led support groups and parent leadership meetings. Helps to engage parents in Community Action Network meetings.

Individuals in this position should be able to work in a fast-paced environment and have the time-management and multitasking skills necessary to perform a variety of duties concurrently. Excellent written, verbal, and collaborative skills.  Speaking with and collecting information from clients and referral sources requires strong interpersonal skills, and attention to detail. Individuals should have knowledge of using various systems including: Microsoft Word, Microsoft Excel, and manage electronic health record systems.

ESSENTIAL RESPONSIBILITIES

  • Demonstrate cultural awareness and sensitivity in working with families from diverse backgrounds.
  • Create and build connections with persons served to assist them in meeting their needs in order to become self-sufficient. 
  • Provide specialized case management and outreach services to individuals with perinatal mental health issues and substance use disorders
  • Assume responsibility for assessing persons served and family strengths, challenges, and needs.
  • Collaborate and build connections with community partner
  • Assist and facilitate parent led support groups as well as parent leadership meeting
  • Help to engage parents through lived experience and support them in contributing to the CAN
  • Attend meetings promptly and actively participates in: trainings, staff meetings, clinical supervision
  • Stay familiar with and make use of the community and resources, both formal and informal within the community and accessing them first before looking to formal resources; 
  • Complete documentation in a timely and accurate manner
  •  Complete all other tasks assigned by Program Manager within given timeframes. 

Minimum Education Level

Bachelor’s degree preferred. Must be actively enrolled in a bachelor’s degree program in counseling, psychology, social work, or related program with proof of enrollment and good standing in a regionally accredited bachelor’s degree program OR have an Associate degree in counseling, psychology, social work or related field plus appropriate experience. Bilingual preferred. Must have reliable transportation, liability insurance and TN driver’s license.

$14.25--$20.00
