Ennoble Care is seeking a full-time, experienced Clinical Care Coordinator specializing in Nephrology/Hemodialysis to join our Tulsa, Oklahoma team. This on-site role supports dialysis patients through thoughtful care coordination, clear communication, and close collaboration with clinicians, patients, families, and partner facilities.

 

Responsibilities

  • Coordinate care for dialysis patients using Ennoble Care’s electronic medical record system
  • Maintain regular patient contact to provide support, coordination, and follow-through on care plans
  • Communicate with clinicians regarding patient needs, changes in condition, and emergent concerns
  • Document patient and partner communications clearly, accurately, and in a timely manner
  • Collaborate with a multidisciplinary care team to support continuity of care
  • Follow documentation standards and escalation protocols
  • Contribute positively to team workflows and daily operations

 

Qualifications

  • Minimum one year of experience working with nephrology/hemodialysis patients
  • Comfortable communicating by phone and in person throughout the day
  • Strong organization skills and the ability to manage multiple patients and priorities
  • Knowledge of basic healthcare terminology, conditions, roles, and care principles
  • Clinical skills including obtaining vital signs and drawing laboratory studies
  • Proficiency with Microsoft Outlook and other Microsoft Office programs
  • Ability to work full-time, Monday through Friday (approximately 8:30am to 5:00pm CST), on-site, in Tulsa, Oklahoma
  • Valid, unencumbered driver’s license, reliable personal vehicle, and current auto insurance, with the ability to provide proof (some travel between facilities may be required)
  • Ability to pass a background check and drug screen
