Overview

Leads a team to achieve excellence in the daily operations of care coordination with a focus on quality, compliance, and member outcomes. You’ll provide coaching and unwavering support for the care management team, adeptly addressing problems, resolving issues, and navigating regulatory and service-related changes. Manages and coordinates the activities of the VNS Health Plans Care Coordination team to ensure member care coordination provided is consistent with Enterprise mission and strategic goals. Partners across the organization to achieve goals and provide each member with comprehensive, community-based care services.

Compensation Range:$98,200.00 - $130,800.00 Annual

What You Will Do 

  • Leads a team of about 15 staff and manages the day-to-day operations that contribute to comprehensive home and community-based care services in accordance with clinical practice standards, state, and federal regulations.

  • Collaborates across multiple departments to achieve quality care, compliance, and member outcomes.

  • Uses software systems including Excel dashboards to manage processes, data, and performance metrics.

  • Coaches team members on productivity and performance, fosters collaboration and decision making.

  • Masters a varying compliance environment to ensure that all policies and guidelines are adhered to.

  • Develops and implements process improvement plans.

Qualifications

Licenses and Certifications:

  • License and current registration to practice as a Registered Professional Nurse in the state of New York, required


Education:

  • Bachelor's Degree in nursing required or

  • Master's Degree in Social Work required


Work Experience:

  • Minimum five years of experience in home care, care management or related field, required

  • Minimum of two years progressive experience in clinical management, required

  • Demonstrated knowledge of NYS regulations regarding Medicare or Medicaid Managed Long Term Care health plans, preferred

  • Experience in a managed care environment, preferred

  • Experience in excel, preferred
