Salary: $109,945.46 - 113,556.22 base salary per annum

About the role:

We have an exciting opportunity for a Clinical Nurse Specialist (CNS2) to work as a Clinical Nurse Navigator in a dynamic team leading our approach to the provision of quality care to clients of the Palliative Care Service.

In this role, you will exercise extended autonomy of decision making and clinical judgement while coordinating the flow and care coordination of patients in the palliative care service. This will include, but is not limited to, triaging referrals, managing waitlists, leading multidisciplinary meetings, collaborating with internal and external key stakeholders, and reviewing data for service improvement.

The Palliative Care Clinical Nurse Navigator is an experienced registered nurse with advanced knowledge of the health needs of patients diagnosed with life-limiting illnesses. They will demonstrate the ability to act as a professional role model and clinical leader, with a high-level capacity for patient advocacy, while working independently and as part of the multidisciplinary team.

About you: We are seeking a skilled and energetic Clinical Nurse Specialist who possesses a passion for person centred care. We welcome applications from people who have experience, or an interest, in palliative care nursing, who can work independently and collaboratively as part of a diverse multidisciplinary team.

Selection Criteria

  • Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia

  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency

  • Relevant post-registration qualifications and at least three years’ experience working in the clinical area of their specified post-graduate qualification

  • Demonstrated ability to work within the interdisciplinary team at a senior level

  • Demonstrated commitment to ongoing professional development and demonstrated effective communication and interpersonal skills

  • Demonstrated effective computer literacy with email and an electronic patient record system

  • Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse

  • Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment
