Job Overview

The RN Care Navigator plays a crucial role in ensuring the highest quality of care for patients. As a key member of the Care Coordination team, the RN Care Navigator works collaboratively with healthcare providers, patients, and their families to facilitate the seamless transition of care throughout the healthcare continuum. This position requires a compassionate, patient-centered approach and strong clinical knowledge to improve patient outcomes and optimize the healthcare experience.

Job Requirements

  • BSN, Master's preferred

  • IL RN License

  • 2 Years of clinical nursing experience, in a hospital setting

  • Certified Case Manager (CCM®),  (CCMC) or a related field required


What you will do:

  • Conduct comprehensive assessments of patients' medical, social, and psychological needs.

  • Collaborate with healthcare providers to develop individualized care plans that address patients' unique needs and goals.

  • Identify potential barriers to care and work to address them proactively.

  • Serve as a liaison between patients, families, healthcare providers, and community resources to ensure a smooth transition of care.

  • Monitor and manage patients' progress throughout their healthcare journey.

  • Facilitate timely referrals and appointments with specialists, home health agencies, or other services as needed.

  • Coordinate and arrange for transportation or other support services when necessary.

  • Educate patients and their families about their diagnoses, treatment options, and available resources.

  • Advocate for patients' rights and preferences within the healthcare system.

  • Empower patients to actively participate in their care decisions.

  • Maintain accurate and up-to-date electronic health records, documenting all interactions and interventions.

  • Prepare reports and summaries to communicate patient progress and outcomes to the healthcare team.

  • Collaborate with the interdisciplinary team to identify opportunities for improving care processes and outcomes.

  • Participate in quality improvement initiatives and contribute to the development of best practices.

  • Ensure compliance with all relevant healthcare regulations and standards, including HIPAA
