Employee Health Plan (EHP) Care Coordinator

EHP Care Coordinators provide intervention to caregivers in the chronic condition program through health coaching, healthcare coordination and health education. EHP Care Coordinators promote Employee Health and Wellness Center services, preferred provider networks, behavioral health services and other benefits.  
 

A caregiver in this position works 8:00AM – 4:30PM. 


A caregiver who excels in this role will:  

  • Collaborate and consult with a multidisciplinary team regarding members' health and perform assessments and evaluations to monitor progress via telephonic and electronic methods. 

  • Contact assigned EHP members per department protocol and interact with members in a professional and courteous manner. 

  • Document all interactions via electronic platform. 

  • Evaluate all relevant data and obtain information while carefully considering the facts surrounding the case. 

  • Utilize motivational interviewing with members to promote lifestyle changes to achieve designated outcomes. 

  • Refer members to their healthcare provider or preferred provider networks, primary care physicians, and other services as needed. 

  • Maintain confidential information regarding the health of plan participant.  

  • Maintain knowledge and adherence to federal and state regulations regarding the confidentiality of such information including HIPAA. 


Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of nursing or behavioral health science.  

  • Current state licensure as a Registered Nurse (RN), Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW).  

  • RNs are required to have multistate license (MSL) or are eligible and willing to obtain within 90 days of hire.  

  • Three years of clinical experience.  

  • Knowledge of chronic condition programs.  

  • Knowledge of and well-versed in all areas of federal and state regulations regarding HIPAA. 

  • Proven commitment to customers that focuses on improvement and enhancement to the service delivery process.  


Preferred qualifications for the ideal future caregiver include: 

  • Bachelor of science in nursing (BSN).

Physical Requirements:

  • Manual dexterity to operate office equipment.
  • May require extended periods of sitting.
  • Good visual acuity through normal or corrected vision and good audio acuity.
