Job Description Summary:
Certified Wellness Coaches work with individuals and groups in a coach centered process to facilitate and empower the coaches to develop and achieve self-determined goals related to health and wellness. Coaches support clients and patients in mobilizing internal strengths and external resources and in developing self-management strategies for making sustainable, healthy lifestyle behavior changes.
Qualifications:
- Bachelor’s degree in Exercise Science, Health Promotion, Counseling, Nursing, Dietetic/Nutrition, Psychology, Social Work or related field required.
- Certification as a Wellness Coach from a nationally recognized organization required. National Board for Health and Wellness Coaching certification preferred.
- Minimum of one (1) year experience facilitating/coaching positive behavior change required.
- Current CPR/AED certification preferred, or must be successfully obtained within sixty (60) days of hire.
- Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
- Commitment to working in a team environment and maintaining confidentiality as needed.
- Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
- Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
- Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.
Job Specific Duties:
- Reviews provider referral and health history questionnaire. Communicates with referring provider for each client with entry, discharge, and untoward event reporting. Communicates and motivates clients in an effort to monitor progress.
- Assesses client’s well-being status, readiness for change and motivation to adopt new health and wellness behaviors.
- Works with client to establish strengths, refine a personal wellness vision, and create program goals appropriate to health status. Adjusts and revises individualized goals and plan in concordance with client progress and needs.
- Implements a range of interventions and skills that is consistent with education/training, certification/licensure and scope of practice.
- Ensures all initial and final outcome assessments are obtained for each client. Documents client records, activities, and coaching sessions per department protocol and in accordance with privacy and HIPAA laws.
- Conducts biometric screenings, finger stick lipid and glucose screens, body compositions measures, and blood pressure. Interprets results and provides recommendations for improving scores.
- Builds professional relationships and provides customized, evidence-based education presentations and seminars for clients, schools, and community members. Topics include a variety of areas, but may not be limited to; body image, bullying, weight management, tobacco cessation, chronic disease risk reduction, physical activity stress, and improved nutrition Accurately gathers scientifically and medically sound information and organizes presentations.
- Assists with facilitating wellness activities and events per department protocol.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
