Bachelor’s degree in Health Education or related field or
High school diploma/GED plus four (4) years of health education experience required.
Special Experience: This position is paid through a grant: UAMS Project Heal: A Hospital-based Violence Intervention Program that requires the person hired to have previous experience of being violently assaulted (i.e., gunshot, stabbing, etc.) and treated in a hospital for penetrating injury and recovery to ensure the ability to build rapport and provide peer support to the program participants by drawing from lived experiences.
Knowledge, Skills & Abilities:
At least 3 years of experience working with health care systems, victims’ services, mental health or substance use treatment programs or community-based programs/organizations.
Experience with recruitment, facilitating collaborative partnerships, client assessment, treatment or action planning, conducting interviews, advising/counseling/mentoring clients, leading group discussions, and participating in health promotion and health education activities.
Intimate understanding of and experience working with the community served by the individuals impacted by community violence.
Ability to initiate and maintain positive working relationships with project staff and other organizations.
Ability and willingness to provide emotional support, encouragement, and motivation to program participants.
Physical Requirements:
Requires a high level of mobility in order to be able to provide outreach at various locations if needed.
Preferred Qualifications:
Experience engaging communities with a high prevalence of violent crime in Little Rock.
Experience working with community-based organizations that provide mental and/or social services to individuals impacted by community violence.
Participation in community violence prevention programs or research.
Experience living or working in Little Rock, AR.
Experience working with diverse groups of people.
Experience working as part of a multi- disciplinary team, collaboratively practicing with health professionals, governmental, and/or community-based organizations.
At least two years of community/peer support or mentoring experience is preferred.
Experience using creative and interactive approaches to engaging and building rapport with clients/participants.
Experience with case management, service referral, and navigation.

Responsibilities:
Operate in a supportive role within an interdisciplinary research team.
Provide peer support, outreach, case management, and navigational services to current and recently discharged patients with violent assault injuries.
Conduct brief bedside interventions, program eligibility screenings needs assessments; develop and maintain follow-up action plans; and co-lead peer group sessions.
Collaborate with Emergency Medicine, Trauma, and Psychiatry healthcare providers, social workers, and community-based organizations to provide follow-up health care and
social services that will maximize patient health outcomes.
Works under the direct supervision of the Program and Clinical Directors.
Knowledge of community-based social, behavioral health, medical and other services, and patient-centered care are requirements of this position.
Additional assistance with appointment scheduling is a requirement of the job.
Specific training in violence prevention services will be provided.
Provides peer support, empowerment, education, and case management services to current and recently discharged patients treated at UAMS for a violent assault.
Recruits patients to the program via bedside and telephone approaches; conducts periodic assessments of patients’ social, medical, and other healthcare needs, and social and economic resources; provides consistent and frequent engagement with participants; conducts home visits when appropriate and necessary.
Educates patients about the health care system and facilitates relationship building between the two.
Documents work with patients through appropriate record-keeping that follows the project’s policies and procedures.
Makes referrals for needed services and facilitates access to community resources.
Co-leads psychoeducational group sessions.
Participates regularly in project team meetings.
Participates/assists as needed in any activities related to the program.
Works in collaboration with partners of the study and attends necessary ongoing training for CHCs.
Advises participants and others regarding health care and other facilities available to them.
Assists participants in utilizing such services.
Makes follow-up contacts when required.
Serves as liaison between the professional staff and the community, including developing relationships with various stakeholders in the social service community.
Telephone calls and outreach to participants as needed, including crisis intervention.
Facilitation client transportation ambulatory patients between their homes and clinics, hospitals, or other social agencies.
May transport staff members to meetings with administration approval; report malfunctions of the vehicle to supervisor. Working as part of the team to enhance the overall patient experience.
Assists in gathering and evaluating data concerning the program.
May perform incidental clerical duties such as keeping records, answering the telephone, arranging participant appointments, requesting medical records, etc.
