About the Role
This position sits within the SVHM Healthcare for the Homeless program and will provide an in reach primary health service to the Elizabeth St Common Ground (ESCG) service run by Launch Housing in partnership with Unison. Whilst the role will primarily be embedded within ESCG, it will also maintain close professional links with SVHM Healthcare for the Homeless department and related services.
Undertake scoping of acute and chronic health needs of those residing at ESCG
Continue the implementation of the Primary Health model of care, in conjunction with SVHM Healthcare for the Homeless department and Launch housing ESCG team.
Work collaboratively with the Launch Housing team to foster a joint understanding of the primary health service at the site
Provide community based clinical nursing assessment of acute and chronic health needs of residents and support residents through a care coordination approach to develop goals and achieve improved health outcomes
Classifications for this position is RN 4B YX11 ($113.646 per annum, pro rata).
Your Contribution
Registered Nurse, with 7 years’ experience in acute/community setting.
Eligible for registration/membership with AHPRA
Current Victorian Driver’s Licence
Advanced clinical skills and experience in an area of mental health, alcohol and other drug use, working with residents with complex psychosocial needs.

About the department name
The ESCG is an Internationally renowned model of housing that focusses on transitioning long term homeless people to a permanent stable home operating from a housing first philosophy. ESCG has been operating in Melbourne for over 12 years. ESCG is a high density 131 Unit mixed tenancies facility located in the heart of Melbourne. It provides on-site case management support to 65 individual residents. Supports and 24/7 Concierge services are provided by Launch Housing with support also provided by other ancillary services and tenancies are managed by Unison.
