Job Description:

SCOPE: The patient navigator acts as a link between patients and care providers to facilitate clinical and supportive care services. The role is that of a single point of contact among referring providers, patients, and other clinical caregivers for the purpose of empowering patients in dealing with the disease and the treatment.

FUNCTIONS:

1. Facilitate Patient and Family Engagement in Care Plans.

2. Coordinate Comprehensive Care and Support Services.

3. Leadership and Quality Improvement.

4. Outreach and Relationship Management.

5. Other Duties as Assigned.

QUALIFICATIONS:

High School Diploma or GED Required.

Associate Degree Preferred.

Minimum 2 years of clinical or non-clinical healthcare related experience.

Experience working in the Oncology specialty, or a related field, highly preferred.
