As a Clinical Field Staff Liaison (RN) the work you do every day makes a difference in the lives of our patients by providing support and acting as a resource to our clinicians in the field. HOW YOU'LL WORK:
You’ll serve as a resource for clinical leadership and clinicians in the field, at your primary location and for other locations within the network. You will facilitate calls to and from field staff as well as use resources to resolve issues and reports anything of concern to your manager. You'll be responsible for all practices and duties within the scope of practice outlined by the state.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care: Ensure Start of Care compliance.
Minimize Patient Risk: Ensure quality and safe delivery of home health services and assist in monitoring outcomes.
Communication: Create relationships with the local agencies and communicate with agency leaders on an as needed basis. Facilitate calls to and from Field staff; using resources to resolve issues and reports anything of concern to manager.
Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations. Review clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude
Patience is a virtue when working with patients, families, physicians, and coworkers
Attention to detail is critical, as is being observant and following directions
Knowledge of MS Office applications including Word, Excel, PowerPoint and Outlook
REQUIREMENTS:
Registered Nurse with current license in the state of Ohio and may require a Registered Nurse license in multiple states, based upon business needs
Minimum of three (3) years of experience as an RN in an acute care setting
Minimum one-year home health experience is preferred
Capable of all physical demands.
