17.46 - 22.20 USD Per Hour

Monday - Friday 8 am to 4:30 pm

What you bring to SBHG:
- High school diploma or equivalent required.
- Associate Degree required
- Bachelor’s degree in a social science or related field preferred.
- (2) years’ experience in a mental health setting Preferred
- Bilingual in Spanish preferred
- Experience with case management services preferred.
How you will make a difference:
- Participates in initial assessments and obtains outpatient treatment provider information, options, and resources.
- Verifies follow-up appointments and collaborates with follow up treatment and step-down services.
- Conducts groups to assist patients with transition to outpatient/community-based services.
- Collaborates with PHF staff and caregivers to plan for and schedule discharge.
- Provide community resources and psychoeducation. Submits discharge packets to referred agencies.
- Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established.
- Transport clients to and from appointments, hospital visits, etc. as needed.
- Develops and maintains relationships with external partners, including DBH and DSS and outpatient providers.
Division/Program Overview:
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 5-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
