Job Title: Community Health Worker : Temporary Role
Department: Healthy Androscoggin
Position Summary:
The Community Health Worker will help individuals and communities adopt healthy behaviors and will conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health. Occasional evening and weekend hours may be required to fulfil the program objectives.
Supervisory Responsibilities:
Duties/Responsibilities:
- Identifies or contacts members of high-risk or otherwise targeted groups such as members of minority populations, low-income populations, or pregnant women. Advises clients or community groups on issues related to improving general health.
- Attends community meetings or health fairs to understand community issues or to build
relationships with community members. Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community. Collects information from individuals to compile vital statistics about the general health of community members.
- Advises clients or community groups on issues related to risk prevention (i.e. HIV), diagnostic screenings such as breast cancer screening, pap smears, glaucoma tests, or diabetes screenings.
- Advocates for individual or community health needs with government agencies or health service providers.
- Assists families to apply for social services including Medicaid or Women, Infants, and Children (WIC). Interprets, translates, or provides cultural mediation related to health services or information for community members.
- Implements and evaluates objectives outlined in grant work plans.
- Maintains positive working relationships with local businesses, agencies (including hospitals and other health care and social service organizations), municipal governments, schools, law enforcement, businesses and other work sites, civic and community groups, media, parents, youth and other community members. Networks on local and state level.
- Maintains confidentiality of information at all times.
Required Skills/Abilities:
- Knowledge of community health resources.
- Understanding of general medical terminology and basic medical procedures.
- Ability to effectively communicate with a diverse population and establish trust.
- Effective active listening skills and social perceptiveness.
- Be strengths-based in your approach to the work.
Education and Experience:
- Minimum High School Diploma or equivalent required Bachelor’s degree in healthcare related field required.
- One to Three years of related experience required.
- Must have lived experience in community being served.
- Experience in: community organizing, community health improvement, prevention planning, public health, health promotion, policy development, youth empowerment, and/or advocacy.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer. May be required to stand for prolonged periods of time, climb stairs, and perform strenuous activity.
- Must be able to lift up to 50 pounds at times.
- Must be comfortable working off-site and in the communities we serve

Essential Duties:
- Provide coaching through motivational interviewing and other evidence-based approaches.
- Advocate for individuals and communities with a goal to build individual community capacity.
- Conduct targeted and community outreach as needed to promote the goals of the program.
- Provide culturally appropriate health education and information.
- Participate in health care promotion activities with individual patients and in communities served.
- Identify, document and address positive social determinants of health-related barriers such as food or housing insecurity, transportation needs etc.
- Assist in the preparation of health and finance related applications as needed.
- Participation in various initiatives and projects in collaboration with diverse community partners.
- Participate/complete in required training programs and development activities.
- Prepare reports and documents as needed or requested.
- Other duties as reasonably assigned.
- Ability or interest in community organizing and assist with outreach activities.
- Schedules and facilitates information sessions and workshops.
- Manage a flexible schedule that may include travel and working outside the traditional office space and times.
- Help disaster survivors understand their current situation and reactions.
- Reduce stress and provide emotional support.
- Assist survivors in reviewing their disaster recovery options.
- Promote the use or development of coping strategies.
- Connect survivors with other people and agencies who can help them in their recovery process.
