Job Description:
The Hospice Registered Nurse On-Call is a trained professional nurse who delivers nursing care to patients assigned to be seen in the evening or after hours. This is accomplished by making regular visits to patients where they reside, providing patient/family clinical and psychosocial needs in accordance with attending physician’s orders and the interdisciplinary team (IDT) plan of care. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, other inter-disciplinary team (IDT) members and, if the patient lives in a nursing home, with the nursing facility staff.
JOB REQUIREMENTS:
- EDUCATION: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- EXPERIENCE: Minimum one (1) year of experience as a nurse in a clinical care setting or home health environment is preferred.
- CERTIFICATION/LICENSURE: Current State License as a Registered Nurse.

- Ability to build rapport and communicate with patients, families, physicians, facilities, and other members
- Demonstrates ability to assess and respond to the needs of patients and families
- Demonstrates ability to cope with family emotional stress and tolerance of individual lifestyles
- Knowledge of current requirements of the Conditions of Participation and third-party payer
- Proficiency in phlebotomy skills and medication administration
- Ability to handle multiple priorities, documentation requirements and deadlines
- Knowledge of healthcare industry is preferred
- TRANSPORTATION: Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
- HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.
KEY ACCOUNTABILITIES:
- Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
- Assesses assigned case load of home health care patients to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team reports, call-in logs, and on-site evaluations.
- Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit.
- Communicates significant findings, problems, and changes in health conditions, environment, or unsafe facility conditions to the clinical supervisor, the physician and/or other personnel involved with patient care.
- Counsels, instructs, and implements the plan for patient safety, utilizing patient information, family, and community resources.
- Participates in the implementation of the plan of care of an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes.
- Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation.
- Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards and guidelines.
- Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor.
- Writes physician orders to cover additional visits and changes to the plan of care.
- Provides those services ordered requiring substantial and specialized nursing skill.
- Follows appropriate preventative and rehabilitative nursing procedures.
- Documents medication regimen and updates medication profile.
- Supervises and provides clinical direction to the Licensed Practical Nurse & the Home Health Aide.
- Instructs and evaluates the Licensed Practical Nurse and the Home Health Aide with special patient care procedures in accordance with the scope of practice and home health care policies of the agency.
- Maintains current knowledge of, and promotes compliance with all federal, state, and local regulations.
- Maintains fully aware of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance.
- Participates in patient care conferences offering clinical suggestions and updates of patient’s progress towards goals.
- Monitors assigned cases to ensure compliance with requirements of third-party payor.
- Implements all available actions to prevent avoidable hospitalization and ER visits.
- Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
- Handles the personal property of patients and their families with care.
- Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
- Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Performs other duties/projects as assigned.
PHYSICAL AND ENVIRONMENTAL DEMANDS:
- Physically demanding, high stress environment
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

- Cope with work involving ill, disabled, and dying patients
- Abide by all occupational safety and bloodborne pathogen protocol to minimize illness and/or injury
- Complete required work daily
- Potentially work irregular hours including call hours if applicable
