Job Description:
Newburyport Territory/Full Time (Monday through Friday 8a-5p)

As we continue to grow in the Newburyport market, we are expanding our clinical team and seeking a dedicated Registered Nurse (RN) – Home Health who is ready to be part of a mission-driven organization that values clinical excellence, autonomy, and meaningful patient relationships.

As a Home Health Registered Nurse, you will manage a caseload of patients and provide comprehensive nursing care throughout their healthcare journey.
Responsibilities include:
Completing skilled nursing assessments and developing individualized plans of care
Performing admissions, routine visits, recertifications, and discharge visits
Evaluating patients' physical, psychosocial, environmental, and educational needs
Coordinating care with physicians, caregivers, and interdisciplinary team members
Monitoring patient progress and identifying changes in condition
Updating and revising care plans to ensure optimal outcomes
Educating patients and families on disease management, medications, and self-care techniques
Maintaining accurate and timely clinical documentation
Helping prevent avoidable hospitalizations through proactive intervention and patient education
Delivering compassionate, patient-centered care while promoting safety and independence
Current, unrestricted Massachusetts Registered Nurse (RN) license
Minimum of one (1) year of nursing experience in a clinical care setting
Ability to commit to at least one full day (8 hours) per week
Valid driver's license, reliable transportation, and current auto insurance
Willingness to travel throughout the assigned territory
Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
