For more than 30 years, Banner Behavioral Health Hospital has provided a nationally recognized, behavioral health care program for children, teens and adults faced with psychiatric, behavioral health, or chemical dependency challenges. We are dedicated to providing safe, confidential and compassionate treatment with customized treatment plans to meet each patient's needs – in the least restrictive therapeutic environment. We also can help patients with co-existing psychiatric and substance abuse problems. 

Our staff provides assessments for outpatient treatment, as well as offering Intensive outpatient programs at both our Scottsdale facility and Chandler office for adults and adolescents with chemical dependence or mental health challenges.

This position will be a Part Time Day Shift Saturday & Sunday 8:30am-5:00pm

POSITION SUMMARY

This position provides comprehensive care coordination for patients as assigned. This position assesses the patient’s plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient’s health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.

CORE FUNCTIONS

1. Manages individual patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

2. Provides assessment, direct psychoeducation, and direct psychotherapy to individuals,    families, and groups. Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.

3. Acts in a leadership function with process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

4. Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes.

5. Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team. Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.

6. Acts in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge plan. Effectively communicates the plan across the continuum of care.

7. Educates internal members of the health care team on case management and managed care concepts. Facilitates integration of concepts into daily practice.

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Banner Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.

PHYSICAL DEMANDS/ENVIRONMENT FACTORS

DP - Typical Direct Patient Care environment: (Nutrition Rep, Chaplain, RN)
Able to stand, walk, bend, squat, reach, and stretch frequently.
Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs.
Needs adequate hearing and visual acuity, including adequate color vision.
Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects.
Able to use proper body mechanics to assist patients in ambulating, transferring in and out of bed, chair or wheelchair.
May be required to lift up to 75 pounds.
Must use standard precautions due to threat of exposure to blood and bodily fluids.
Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.

May require periodic use of personal computer.

MINIMUM QUALIFICATIONS

Requires master’s degree in social work or related field or equivalent experience.

Requires a proficiency level typically achieved with 5 years clinical experience and as required by state law, a licensure in at least one of the following categories: social work, professional counseling or marriage and family. (Certification requires two years post graduate experience in a mental health setting or equivalent combination of education and documented supervised clinical experience.)

Positions providing direct services to substance abuse program patients must also possess board licensure in addictions as required by state.

Must have a working knowledge of care management, hospital and community resources.

Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format.

Employees working at BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working at Banner Behavioral Health Hospital or BTMC Behavioral must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working in the Banner McKee Senior Behavioral Health Inpatient Unit must possess a Colorado Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
