Mental health complex care coordinators (MHCCCs) are a valuable member of the hospital-based clinic care team. MHCCCs provide collaboration, consultation, and care coordination to patients across the lifespan who receive care within the hospital-based clinics (Abbott Northwestern, United, Unity, West Health, New Ulm and Cambridge). The primary role of the MHCCCs is to support providers and patients by helping to connect patients with outside resources that would be beneficial to their overall well-being. While they are not case managers, MHCCCs can talk with patients, families, and others involved in the patient’s life to support connection with community, state, and other resources. In addition to their role as care coordinators, our MHCCCs are LICSWs, which allows them to complete diagnostic assessments (if needed for accessing resources). In some instances, the MHCCCs may also help the care team by calling patients between visits if there are concerns that the patient may have mental health care needs that will arise before the next scheduled visit with their provider.

Job Description:

Allina Health has a robust Mental Health and Addiction Clinical Service Line that offers the full continuum of care including integration, specialty outpatient clinics, addiction and co-occurring services, partial hospitalization and day treatment, and inpatient care. There are more than 290 mental health and addiction clinicians dedicated to the highest quality patient care across 45 primary care clinics, 6 integrated psychiatry locations, 4 hospital-based clinics, 15 partial and day treatment programs, 15 Allina and affiliated Emergency Departments, and 7 hospitals with 270 inpatient beds. The Mental Health and Addiction Clinical Service Line is led by both psychiatry and psychology clinical leadership, in partnership with operations to advance mental health care for patients across Allina Health.

Principle Responsibilities 

  • Coordinates care for patients via in person, telehealth, and telephone contacts 
    • Assesses conditions related to crisis and life events 
    • Screens for basic health needs and align services to assist. 
    • Identifies needs for community or public agency services and offers referral resources 
    • Follows-up with patient and care team about identified resources and offers support as needed to help patients access resources 
    • Follows mandated reporting guidelines 
    • Ensuring documentation meets regulatory and compliance standards 
    • Other duties as assigned 
  • Collaborates with care team and supports patient’s mental health needs 
    • Respond to care team messages regarding patient care needs 
    • Support patient care coordination needs between scheduled provider visits including, but not limited to: checking in about patient’s status, providing brief therapeutic intervention offering crisis resources as needed, coordinating with provider/care team if urgent but non-life-threatening concerns arise, following holds process if life-threatening concerns arise 
    • Collaborate with patient provider to ensure updated DAs are completed as required by DHS 
    • Conduct brief psychotherapy visits to support bridging care when their psychotherapy provider is out of office 
    • Conduct brief psychotherapy to support re-engagement in care with psychology and psychiatry when patients have repeat no-shows or late cancellations 
  • Support the hospital-based clinics 
    • Collaborate with leadership to develop and implement programs and workflows that support patient care 
    • Create & provide materials & trainings for interns and new and current employees on policies, procedures, laws, & departments.   
    • Promote the advancement of the MHCCC group by co-facilitating, organizing, and documenting team meetings 

Required Qualifications

  • Master's degree in social work required


Preferred Qualifications

  • 2 to 5 years in licensed practice


Licenses/Certifications

  • Licensed Ind Clinical Social Worker - MN Board of Social Work required upon hire
  • BLS Tier 1 - Basic Life Support - Multisource required within 180 Days
