We’re looking to find our founding Care Navigator / Community Liaison for our MediCal programs. This individual is responsible for supporting members in improving their overall health through outreach and engagement activities. This role involves navigating the healthcare system, advocating for members, and collaborating with medical teams to ensure seamless care, and utilize evidence-based approaches to promote member participation

Job Responsibilities

Patient Recruitment:

  • Identify and engage potential members eligible for Guaranteed's services

  • Collaborate with hospitals, nursing homes, churches, community organizations, and doctor offices to source referrals

  • Educate case managers and healthcare professionals about eligibility criteria and referral processes.

Community Outreach:

  • Serve as a liaison between our organization and community partners.

  • Build and maintain strong relationships with healthcare providers, community leaders, and organizations.

  • Represent the organization at community events, meetings, and forums.

Care Coordination:

  • Provide recurring care coordination for members

  • Facilitate referrals to community support services based on members needs.

  • Coordinate care plans with patients' primary care physicians (PCPs) and other healthcare providers.

  • Monitor patient progress and adjust care plans as necessary.

Documentation & Reporting:

  • Maintain accurate and confidential records of patient interactions and care plans.

  • Prepare regular reports on outreach activities, patient recruitment, and care coordination outcomes.

Skills and Toolset You’ll Need to Bring

  • CHW Certification, or ability to complete Certification within 6 months of employment

  • Proven experience as an in-person marketer or community liaison.

  • Demonstrated ability to work effectively with community partners, healthcare professionals, patients, and families.

  • Understanding of and sensitivity to multi-cultural communities.

  • Bilingual in Spanish preferred.

  • Valid California Driver's license, valid automobile insurance and a clean driving record.

  • Willingness to work Monday-Friday 8:30am-5:00pm, primarily in the field.
