We’re looking to find our founding Community Health Worker for our MediCal programs in Santa Clara County.

The Community Health Worker at Guaranteed is responsible for supporting members in improving their overall health through outreach and engagement activities. This role involves navigating the healthcare system, advocating for members, and collaborating with medical teams to ensure seamless care, and utilize evidence-based approaches to promote member participation

Compensation: At Guaranteed, we aim to offer equitable and competitive compensation that demonstrates the value we place in our employees and our appreciation for the work they perform; the hourly range for this role will fall between $19-$25 per hour

Job Responsibilities

  • Assist Members in navigating the healthcare system by following up with hard-to-reach Members both by phone and in person.

  • Engage with Members using evidence-based approaches like Motivational Interviewing to promote collaboration with their medical/behavioral teams.

  • Deliver health and wellness information in an easily understandable manner and provide information on Member benefits and services.

  • Advocate for Members in various settings including home, community, and provider organizations.

  • Build and maintain positive relationships with Providers through effective communication.

  • Understanding and share the culture and preferences of Members and their community.

  • Collaborate on Member care issues with care team members and consult with Lead Nurse Care Manager for clinical actions.

  • Engage in quality improvement initiatives and projects to improve outcomes for Members

  • Ensure documentation is accurate and compliant with regulatory requirements and accreditation standards.

  • Participate in team meetings and other necessary activities.

Skills and Toolset You’ll Need to Bring

  • High school diploma or GED required.

  • Two years of experience as a Community Health Worker, Promotora, Health Navigator, or in community outreach, customer service, or a medical office, or a Behavioral Health/Substance Use Disorder program.

  • Knowledge of Medi-Cal/managed care environment.

  • Understanding of and sensitivity to multi-cultural communities.

  • Completion of an approved Community Health Worker program or completion within six months of hire.

  • Bilingual in Spanish preferred.

  • Valid California Driver's license, valid automobile insurance and a clean driving record.

  • Willing and able to work Monday-Friday 8:30am-5:00pm, primarily in the field.
