Key Position Details:

  • .5 FTE (40 hours per two week schedule)
  • No weekends
  • Benefit-eligible position

Job Description:

Provides services to clients currently employed in the community so they may retain their employment. Supports clients wherever the job is, learns it along side the individual and provides whatever reasonable supports the person may need to succeed. Supports the employer department's goals of placement and retention.

Principle Responsibilities

  • Supports placed individual on their job site, working from home or in the community.
    • Learns the job along with the individual.
    • Develops accommodations and compensatory strategies for that individual's needs.
    • Sets up workstations.
    • Provides and coordinates additional community resources as needed by the client.
    • Prepares initial assessments, individual support plans, functional limitations, case notes, task analysist and verification forms.
  • Support employer of placed individual.
    • Acts as the liaison between client and employer.
    • Provides specific disability information and training to employer as needed.
  • Other duties as assigned.


Required Qualifications

  • Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
  • 0 to 2 years of working with people with physical, cognitive or mental health disabilities


Preferred Qualifications

  • High school diploma or GED
  • Associate's or Vocational degree Occupational Therapy Assistant or related field
  • Bachelor's degree Physical Therapy, Occupational Therapy or related field
