Position Highlights:

  • Location: LOH- Naperville
  • Part Time- 20 hrs /week
  • Hours: 9a-5:30p weekdays; 8:30a-5p weekends

A Brief Overview:
Provides care coordination and assists with discharge planning services to patients who are in need of ongoing behavioral health consults in the EEH emergency departments or behavioral health inpatient care following discharge from the emergency department. Coordinates Psychiatrist consults and discharge plan at the direction of the patient's treatment team. Obtains appropriate releases, communicates with other care providers/hospitals and collaborates with patients and families and the treatment team to arrange and document a comprehensive discharge plan.

What you will do:

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical, social and psychological needs of patients served and the ability to respond appropriately to those needs.

  • Provides discharge planning services for identified patient as directed by the physician, Assessment Specialist and emergency department treatment team. This may include scheduling outpatient appointments with other facilities, transfer to other hospitals or to Linden Oaks for inpatient or outpatient services.
  • Obtains Release of Information from the patients and families.
  • Assists with Coordination of Psychiatric consults.
  • Ensures all behavioral health insurance benefits have been obtained to help facilitate appropriate referrals and placement.
  • Meets with the patient to explain their options as covered by their insurance and discusses same with the treatment team.
  • Continuously communicates discharge plan progress to patients, patient families and staff in order to help patient with adjustment to hospitalization and to facilitate discharge plan.
  • Maintains accurate and timely medical records including progress notes and after visit summaries.
  • Remains abreast of agencies, professional organizations, and other resources that will assist patients and families in providing continuity of care after discharge and directs patients and patient families to same, as appropriate.

What you will need:

  • Bachelors Degree Social Work Required
  • 1 Year of experience in a behavioral health care setting, preferably acute care. And
  • 1 Year of previous social work experience
