Key Responsibilities:

• Responsible for the direct social work case management services to PACE participants. Provides social services support to include participant screening, case management, counseling, and referral.
• Participate in the Interdisciplinary Team’s initial assessments, care planning and ongoing re-evaluations of participant’s case.
• Maintain current written case management records, including periodic reassessments of program participants.
• Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants.
• Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
Knowledge & Skills:
• Excellent communication; via phone, in person and email.
• Requires individual therapy skills geared to geriatric patients, analytical thinking and problem solving abilities.
• Attention to detail & ability to handle confidential information following HIPPA guidelines.
• Must be flexible to work in other locations if needed.

Educational/Training Requirements:

  • Masters

Certificates, Licenses, and Registration:

  • LMSW/LISW
  • Must be licensure eligible: LMSW, LISW
  • Must possess a valid Florida driver's license, current auto insurance, and reliable transportation.

Experience Requirements:

  • Two years experience in a health related area desired.
  • One year of elder care experience is required.
  • Position requires individual therapy skills geared to geriatric patients, analytical thinking and problem solving abilities.

Position Specific Responsibilities (not all inclusive):

  • Provide individual and family counseling, develop and lead group counseling and activities.
  • Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants.
  • Encourage effective relationships amongst staff geared to the building and maintenance of a cohesive team.
  • Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
  • Participate in program and policy development of the social work component of the program.
  • Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
  • Consistently represents Hope with integrity and professionalism.
  • Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.

  • Assist with special projects, as requested.
