Summary of Job Duties:

Shift: Weekdays - No weekends/Holidays/Call

Performs general nursing functions, such as rooming patients in the clinics and in the patient homes, administering medications, collecting vital signs, assisting physicians with procedures, and phlebotomy. Schedule appointments, returns calls to patients, assists with medication refills and provides patient education. May also perform various health screenings, prior authorizations for medications, imaging, procedures, and other duties as assigned.


Qualifications:

Minimum Qualifications:

  • Must be a graduate from an accredited LPN or LVN school

  • Licensed as an LPN in the State of Arkansas. 

  • Microsoft Office Suite experience and have knowledge of electronic medical records.  

  • Excellent customer service skills. 

  • Valid Drivers License

Preferred Qualifications:

  • One to three years experience working in a busy clinic setting 

  • Phlebotomy skills

  • Previous Home Health experience


Additional Information:

Responsibilities:

  • Works with an interdisciplinary team to facilitate patient care in an outpatient setting.

  • Provides direct case management and assists client services for patients and families.

  • Performs assessments of psychological, financial, and other patient and family needs.

  • Plans and implements care plan, screens new clients for stress, and provides emotional support.

  • Refers patients to counseling and other levels of care and resources.

  • Explains community outreach programs available to patient and families

  • Provides ongoing support for patients as needed.

  • Driving company van to patients' homes.

  • Performs other duties as requested.
