Job Description Summary:

This position is responsible for providing education and support to screening and patients. Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. This position exists in various departments throughout OhioHealth system of hospitals and healthcare services.

Minimum Qualifications:

High School or GED (Required)

MINIMUM QUALIFICATIONS
High School or GED
Field of Study: Social Worker, Licensed Social Worker
Field of Study: Chemical Dependency Counselor
Field of Study: Community Health Worker with 1-3 yrs. exp. in Public Health
Field of Study: and/or Addiction Treatment
Years of experience: 1 - 3
years public health / addiction treatment experience

SPECIALIZED KNOWLEDGE
Applicable public health / addiction treatment knowledge expertise. Desirable certification may vary based on the department in which the job exists.

DESIRED ATTRIBUTES
Bachelor of Science (BSN) from a four-year college or university preferred; Community Health - Addiction Treatment experience.

75%:
Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to patients and families.

Participates in quality control and assurance.
10%:
Participates in process improvement activities including collaboration with community organizations.
10%:
